All students participating in school-sponsored sports (grades 5-12) must have a physical on file at FACS. Physicals must be completed annually, with a new sports exam forms placed on file with FACS each year, prior to the student's first scheduled game.
Physicals may be completed with your family physician at any time prior to FACS team participation. The Little Clinic at Kroger also offers cost- and time-efficient athletic physicals.

Prior to getting a physical, please download the sports exam form below and take it with you to the exam. After the exam, please take the completed form to the school office.
